Form 3 provides you with a tool to communicate with classroom teachers and other members of the team around the child about the access that children have to learning materials and teaching methods. It covers near and distance access through vision, touch and hearing. This form should be filled out following completion of Form 2: Use of Sensory Channels.
In the case of a pupil who is just about to enter a formal education setting (eg, a nursery or reception class) the QTVI will use the data collected about the use of sensory information to generate guidance about how the pupil is likely to access the learning media and what teaching methods will most benefit the pupil.
In the case of children already in school completion of this form will be based upon observations of the pupil's performance in learning situations.
On this form, the QTVI should indicate (with a cross) all the senses that the pupil will be able to use effectively to access learning materials or teaching methods near to or at distance.

The QTVI should work very closely with the classroom teacher and other team members to interpret the information on this form. Each professional brings an important perspective to the task of interpreting this form.
Given the low incidence nature of this population, it is unlikely that the classroom teacher will have had much experience of working with a pupil who is visually impaired. Sitting down with the class teacher and discussing the information on Form 3 provides the perfect opportunity to talk about the teaching methods and learning materials that will best help the pupil.
Similarly, it is likely that the QTVI will not be aware of the variety of teaching methods and learning materials that are used in a specific classroom. A meeting to discuss the information on Form 3 can expand the QTVI's support for real-life learning situations.
