Form 6 is designed to be used after the confirmation of the intial learning media decision. It is completed annually throughout a pupil's school years to ensure ongoing attention to learning and literacy media needs based on a pupil's progress.
The process of annual, continuing assessment will help make sure that the pupil will leave school with the necessary tools to participate effectively in literacy activities in a variety of vocational, social and recreational environments. This phase of Learning Media Assessment may suggest continuation of the pupil's learning programme as it was originally planned, adding supplemental literacy tools, or changing the primary literacy medium.
Make sure to involve all members of a pupil's educational team to gather information that will be included on Form 6. Different members of a pupil's team (including, when appropriate, the pupil him- or herself) provide important perspectives that may have an impact on learning and literacy media decisions.
Form 6 contains five sections:
Each section contains several pieces of information that must be gathered in order for the pupil's educational team to have the necessary data to make informed decisions about the pupil's learning media needs.
You need to take a holistic view of the results of the continuing assessment. Individual assessment results should not be considered in isolation.
The team should carefully consider the appropriate action and plan the pupil's programme accordingly. Please note that if a problem area is identified as part of the continuing assessment phase, it does not mean that a change in literacy medium is an automatic response. Such decisions should be made deliberately, considering all available objective data.
