Phase I Stage 2 confirms that the initial decision is a correct one. It lasts until the child has reached a level of attainment in literacy that will allow for the more formal monitoring undertaken in Phase II.

Form 4: Early Literacy Acquisition Sensory Skills Checklist
If, during Stage 1, the educational team finds that the child has had limited opportunities in developing sensory skills, this form can be used in subsequent years to track the child's access to sensory opportunities. In this way, Form 4 can support teams in deciding if lack of progress in literacy is due to inappropriate media decisions or inadequate opportunities for sensory exploration.
This form will be completed sometime during the reception year when the child has established the beginnings of formal letter and word recognition. it will then be regularly updated through the early literacy phase until the child's literacy skills are established. It will be completed by the QTVI with input form the class teacher/LA.
